Dear prospective new customer,

If you are interested in opening up a new line of credit with our company, it is mandatory that our Application For Credit Form be used.  Substitutions to our form will not be accepted or processed for consideration.

If your business is located in the State of California, and you wish to be tax exempt, a current resale certificate must be on file with Alhambra Rubber. A resale certificate form is provided here for your convenience Resale Certificate Form

These forms have been formatted to allow typing directly to them before printing.
